What this notice covers

This notice covers MuirHealthData’s market-validation survey and separate demonstration-request form. It does not cover the handling of patient or clinical data in a future MuirHealthData product or service.

Information collected

The market-validation survey does not ask for your name, email address or organisation name. It asks about clinical documentation, registry workflows, centre onboarding, costs and related experience.

Please do not include patient information or details that identify you, another person or an organisation in free-text responses.

The separate demonstration-request form asks for your name, professional email address and permission to contact you to arrange the demonstration you requested. It does not request permission to send marketing or product updates.

How we use information

Survey responses are considered in aggregate to understand clinical-registry workflows and constraints.

Demonstration-request details are used only to arrange the requested demonstration. The survey and demonstration-request forms are separate, and we do not ask for a survey-response identifier in the demonstration-request form.

We do not sell this information or use a demonstration request for marketing without separate permission.

Google Forms and international processing

Google Forms and Google Workspace provide the form and storage tools for these responses. Google may process information in countries outside your own under its privacy terms and policies: https://policies.google.com/privacy

We limit access to responses to people who need it for the purposes described in this notice.

Retention

We retain survey responses only while they are needed for market validation, then delete or de-identify them. We retain demonstration-request details until the request is completed, you withdraw permission, or the details are no longer needed for that request.

Your choices and rights

You may decide not to provide information, ask about information held about you, ask us to correct or delete it, or withdraw permission for demonstration contact. Contact us at contact@muirhealthdata.com.

Where applicable law gives you additional privacy rights, we will respond to requests under that law.

Changes to this notice

If MuirHealthData is incorporated, the new company may become responsible for information collected under this notice. Before using your information under the new company, we will update this notice and tell affected demonstration-request contacts.

This will not change the purpose for which we use your information.

We will update this notice if our information-handling practices or operating structure changes.
